How does the admissions process work?
Heritage Home admits residents Monday through Thursday, 9:00 a.m. to 2:00 p.m., and Friday, 9:00 a.m. to noon. Admissions are based on the availability of appropriate Medicare, Medicaid and private-pay vacancies.
Prior to admission, each resident must have documentation of a current diagnosis, preadmission screening and resident review (PASRR), medical history and physical examination by his or her physician. A person with active tuberculosis can’t be admitted, so all residents must either submit a current chest X-ray showing no active tuberculosis or have the two-step tuberculin skin test administered and read. Admitting orders, including diet, medications, treatment and recommended activities must also be included. Recent CBC, RPR, SMAC test, urinalysis and other lab work are also helpful, if they’re available. The physician must specify the appropriate level of care, allergies, rehabilitation potential and a summary of a course of treatment from the discharge facility.
